These Mobile/Text Messaging Program Terms (“Text Terms”) apply to participation in the Preveon Specialty Pharmacy Mobile/Text Messaging Program (“Program”). By enrolling in or otherwise consenting to participate in the Program, you agree to these Text Terms.
1. PROGRAM NAME AND DESCRIPTION
The Preveon Specialty Pharmacy Mobile/Text Messaging Program allows patients and other authorized individuals who have opted in to receive text messages from Preveon Specialty Pharmacy (“Preveon,” “we,” “us,” or “our”).
Depending on your relationship with Preveon and the services you receive, messages may include:
- Prescription refill reminders;
- Prescription or order status notifications;
- Medication delivery or shipment notifications;
- Appointment or scheduling reminders;
- Requests to contact the pharmacy;
- Patient service notifications;
- Pharmacy service updates;
- Administrative communications; and
- Other communications related to your pharmacy services.
Participation in the Program is voluntary.
2. CONSENT TO RECEIVE TEXT MESSAGES
By providing your mobile telephone number and opting in to the Program, you consent to receive text messages from or on behalf of Preveon at the mobile number you provided.
Consent to receive text messages is not a condition of purchasing goods or receiving pharmacy or healthcare services from Preveon.
You represent that you are the subscriber or customary user of the mobile telephone number provided or that you otherwise have authority to consent to receive messages at that number.
If your mobile number changes or is reassigned, you agree to notify Preveon or opt out of the Program.
3. MESSAGE FREQUENCY
Message frequency varies depending on your prescriptions, pharmacy services, refill activity, delivery status, appointments, and interactions with Preveon.
You may receive recurring messages while enrolled in the Program.
4. MESSAGE AND DATA RATES
Message and data rates may apply.
Your wireless carrier’s standard messaging, data, and other rates may apply to messages sent or received in connection with the Program.
Preveon is not responsible for charges imposed by your wireless carrier.
Please contact your wireless carrier if you have questions regarding your messaging or data plan.
5. OPT-OUT — STOP
You may cancel your participation in the Program at any time.
Text STOP to stop receiving text messages from the Preveon Specialty Pharmacy Mobile/Text Messaging Program.
After you send STOP, you may receive a confirmation message indicating that you have been unsubscribed. After processing your opt-out request, you will no longer receive text messages from the Program unless you subsequently provide consent to enroll again.
Opting out of text messages does not affect your ability to receive pharmacy or healthcare services from Preveon. We may communicate with you through other appropriate methods when necessary to provide your pharmacy services.

7. PRIVACY AND USE OF MOBILE INFORMATION
Preveon respects the privacy of information provided in connection with the Program.
Mobile information, including your mobile telephone number and text messaging opt-in or consent information, will not be sold or shared with third parties for their independent marketing, advertising, or promotional purposes.
Information may be disclosed to service providers that assist Preveon in operating the messaging Program or providing pharmacy services, provided such disclosure is consistent with applicable privacy and healthcare requirements.
Information may also be used or disclosed when permitted or required by law.

8. HEALTH INFORMATION AND TEXT MESSAGE SECURITY

By enrolling in the Program, you acknowledge the inherent risks associated with receiving text messages on a mobile device, including the possibility that messages may be viewed by individuals who have access to your device or telephone number.
Do not use text messaging for medical emergencies.
If you are experiencing a medical emergency, call 911 or seek emergency medical care immediately.
9. CARRIER AND SERVICE AVAILABILITY
Delivery of text messages is subject to effective transmission by your wireless carrier and network provider.
Preveon does not guarantee that text messages will be delivered or received within a particular period of time.
Carriers are not liable for delayed or undelivered messages.
The Program may not be available through all wireless carriers, devices, or geographic locations.
10. NO GUARANTEE OF DELIVERY
Text messages should not be relied upon as the sole method for receiving urgent information concerning your prescription, medication, treatment, or pharmacy services.
If you have an urgent pharmacy-related question, contact Preveon directly using the appropriate pharmacy telephone number.
11. CHANGES OR TERMINATION OF THE PROGRAM
Preveon may modify, suspend, or discontinue the Program or these Text Terms when permitted by applicable law.
Any material changes will be communicated or posted as appropriate.
Your continued participation in the Program after changes become effective constitutes acceptance of the revised Text Terms to the extent permitted by law.
